What this website collects

The consultation request form collects a name, an email address or a phone number, an optional treatment you are asking about, and anything you write in the message box. That is all it collects, and it is deliberately the minimum needed to reply to you.

The form asks for no medical information and you should not put any in it. A website form is not a secure medical channel. Your medical history is taken at the clinic.

What we do with it

We use it to reply to your enquiry and to arrange a consultation. We do not sell it, we do not use it for marketing unless you ask us to, and we do not pass it to anyone outside the clinic except the provider that delivers the form to us.

What the clinic holds separately

Clinical records created at an appointment are held under a different regime from this website, with its own retention period and its own lawful basis. Describe that regime here.

Analytics and cookies

Describe what this site uses, if anything. As it ships, this template sets no cookies and loads no analytics script. If you add either, say so here before you publish.

Your rights

You can ask what we hold about you, ask for it to be corrected, and ask for it to be deleted where the law allows. Health records are usually subject to a minimum retention period, so say what yours is.
